Q: How do I obtain a birth certificate?
A: The City Clerk's Office will have birth records of anyone whose parents lived in Haverhill at the time of the birth. A certified birth certificate is available for $10.00 at the City Clerks office or $15.00 by mail. The certified form is required for passports, immigration and other legal purposes.

Q: How do I get a copy of a birth certificate?
A: Certified copies of birth, death and marriage records are available at the City Clerk's Office in Haverhill City Hall and the state Registry of Vital Records and Statistics. You can obtain a copy of your birth certificate from the City Clerk's office in two ways:

  1. By coming to City Hall, City Clerk's office
  2. By mailing a request to the Clerk's office

A request by mail will be processed and sent out the day it is received at the Clerk's Office. You must make sure of 4 things:

  1. Your bank check or money order should be made payable to: City of Haverhill
  2. Enclose the payment along with your written request
  3. State your full birth name and date of birth on the request
  4. Send a self-addressed stamp envelope
