A: The Clerk's Office will have death records of any death that occurred in Haverhill and copies of records of Haverhill residents whose deaths have occurred within the Commonwealth of Massachusetts and used Haverhill as their address at the time of the death.

Q: How can I get a copy of a death certificate?
A: Certified copies of birth, death and marriage records are available at the City Clerk's Office in Haverhill City Hall and the state Registry of Vital Records and Statistics. Assuming the death certificate is on file in the Haverhill City Clerk's office, you can:

  • Come in to the City Clerk's Office and request one in person
  • Request a copy by mail
