City Clerk - Death Certificate
The Haverhill City Clerk issues certified copies of the records of death of any person whose death occurred within the city limits of Haverhill, MA and for any Haverhill resident who passed away in any other Massachusetts community. If you are not certain whether a death record is filed with the Haverhill City Clerk, call the office for confirmation at (978) 374-2312 before ordering online. The City Clerk’s Office is open weekdays from 8:00 AM to 4:00 PM, Eastern Standard/Daylight Time.
Certified copies cost $15.00 each
Certificates will be mailed the same day if the order is placed by 1:00 PM or the following business day if the order is placed on a holiday or weekend.
